How to Transfer to Providence Healthcare of Georgia

Call PHG and speak with our client Intake Team, who will walk you through the entire transfer process and answer any questions you may have.

Care Assessment & Documentation
Our team will:

Submitting the Transfer Request

How long does a Client transfer take?
Every Medicaid program and insurance provider is different, but most transfers are completed within 30 days. In many cases, services with PHG can begin on the first day of the following month after your request.

Can a client transfer be denied?
No. By law, a client transfer cannot be denied. Medicaid clients sign a Freedom of Choice form, which states that they are not under contract and have the right to switch providers at any time, for any reason.

What happens after my transfer is approved?
Once your transfer is processed, Providence Healthcare of Georgia will receive an electronic authorization to begin care.
During this time:
- Our scheduling team will coordinate caregiver placement.

- Your Clinical Supervisor will be available for any questions regarding medical care and ongoing support.
