Where Care Meets Home

We understand the journey of caring for a medically fragile child does not stop when you leave the hospital. In addition- most parents do not feel comfortable to leave their child in the hands of unskilled individuals, so rest is limited.

This is why Providence Healthcare of Georgia partners with physicians and skilled nurses to ensure that pediatric care continues within the home- not only to meet the child’s medical needs, but to also lighten the daily load for families.

GAPP Provider

At No Cost

The Georgia Pediatric Program (GAPP) is a medicaid program that covers the cost of all skilled nursing care and personal care support. 

Parents have the option to be trained and paid as the personal care support if they prefer to provide the service themselves.

Our Services

Our licensed pediatric nurses (LPN/ RN) deliver expert, in-home care designed to meet each child’s medical needs while offering families peace of mind and support.

Examples Include:

  • Administering medications and treatments
  • Monitering vital signs and overall health 
  • Managing feeding tubes, tracheostomies, or ventilators
  • Providing would care and infection prevention
  • Coordinating with physicians and therapist on care plants
  • Educating families on daily care routines

Our certified nursing assistants (CNAs) and personal care aides (PCAs) help with daily activities so children can be comfortable, safe, and supported at home.

Examples Include:

  • Assisting with bathing, dressing, and grooming
  • Helping with mobility and transfers
  • Supporting meal preparation and feeding
  • Maintaining a clean and safe environment
  • Offering companionship and emotional support
  • Encouraging independence in daily routines

Our licensed nurses support children within the school setting, ensuring their medical needs are met so they can focus on learning and enjoying their school day.

Examples Include:

  • Administering medication during school hours
  • Monitoring chronic health conditions (such as diabetes or asthma)
  • Responding ot health concerns or emergencies 
  • Providing tracheostomy or ventilator care in school
  • Coordinating care with teachers, staff, and families
  • Supporting attendance by helping children safely remain in class 

Our nurses make the transition from hospital to home smoother by ensuring continuity of care, reducing stress for families, and helping children recover safely in a familiar environment.

Examples Include:

  • Coordinating discharge plans with hospital staff and physicians 
  • Setting up medical equipment and supplies at home
  • Reviewing medication schedules and administration with families
  • Monitoring recovery progress and vital signs
  • Educating families on new care routines or treatments
  • Providing follow-up support to prevent hospital readmissions
